Course Enrollment Agreement

By enrolling in a continuing education course offered by RejuvaScalp LLC, you ("Student") agree to the following terms:

1. Course Participation Terms

• I am enrolling in this course for my own professional development.
• I understand that this course is for educational purposes only and does not confer professional licensure.
• Only licensed professionals are eligible to perform the services taught; others must enroll in beauty school or an approved apprenticeship.

2. Course Materials & Usage

I acknowledge that all materials provided (e.g., handouts, slides, videos) are the exclusive property of RejuvaScalp LLC. I agree not to:

• Copy, reproduce, or alter any part of the materials
• Use materials for any teaching or training
• Share, sell, or publish them (including online)
• Record or photograph any part of the training without written permission

3. Intellectual Property and Non-Compete

• I acknowledge and agree not to teach, host, or offer any courses, workshops, or training programs related to FSA/HSA compliance and NPI registration for a period of three (3) years following the completion of this course.
• I understand that all course content is protected under U.S. copyright law and agree not to violate these protections.

4. Legal & Financial Terms

• All payments are final, non-refundable, and non-transferable.
• I may reschedule or cancel only if I provide 7 days’ notice before the class date; otherwise, the payment is forfeited.
• I agree to indemnify and hold harmless Guslene Bubak and RejuvaScalp LLC for any claims or legal costs arising from misuse of course materials.
• If legal action is taken due to my breach of this agreement, I agree to pay all legal fees if I lose.

5. Video & Photo Release

By enrolling, I grant RejuvaScalp LLC permission to:

• Use photos, videos, and recordings of me for promotional and educational purposes
• Publish my image and voice across media platforms, including social media and websites
• I waive any rights to approve or inspect the final content
• I release RejuvaScalp LLC from any claims relating to usage or editing of such content

6. Non-Disclosure Agreement (NDA)

I agree to maintain the confidentiality of all proprietary information shared during this course, including business methods, pricing, client lists, and course techniques. I will not:

• Disclose or use this information outside of personal reference
• Circumvent RejuvaScalp LLC by contacting vendors, clients, or leads learned through this course for five (5) years
• Breach confidentiality agreements or share course content with third parties

7. Digital Signature & Acknowledgment

By continuing with enrollment and submitting payment, I acknowledge and agree that:

• This digital acknowledgment is a legally binding signature
• I have read and understood the terms in full
• I accept all course terms, legal disclaimers, and non-disclosure provisions
